Used primarily in the United States, the universal numbering system numbers permanent teeth from one to 32, starting from the upper right molars and moving around the mouth. It’s straightforward to learn with the right support and is widely recognised in American dental practices.

Fédération Dentaire Internationale (FDI) System

The FDI World Dental Federation notation (ISO 3950) is the internationally recognised system for identifying teeth and is widely used in Australia. Each tooth is assigned a two-digit number: the first digit indicates the quadrant and the second indicates the tooth’s position within that quadrant. Permanent teeth use quadrants 1 to 4 and positions 1 to 8, while primary teeth use quadrants 5 to 8 and positions 1 to 5.

Palmer Notation Method

Developed in the 19th century, the Palmer system uses a special right-angle grid symbol to indicate quadrants. Each quadrant uses numbers to identify permanent teeth and letters to identify primary (deciduous) teeth.  

Some orthodontists and oral surgeons still use the Palmer notation; however, the FDI system is much more common nowadays in general practice. 

How Dental Charting Works

Regardless of the tooth numbering system a practitioner uses, the charting process follows a consistent sequence from start to finish: 

  • Dental examination: The practitioner conducts a visual and physical assessment of the mouth, examining each tooth and its surrounding gum tissue.
  • Identify teeth: Using the practice’s preferred numbering system, the dental professional identifies each tooth and locates it within the chart.
  • Record dental conditions: Practitioners then document their findings using standard symbols and notations. The information recorded might include caries, restorations, missing teeth, periodontal measurements and other clinically relevant details.
  • Update records:The chart is saved as part of the patient’s clinical record, ensuring it remains accessible for future appointments and ongoing care.

Dental Charting Symbols and Notations

  • Caries: Marked by a red outline or shading in the affected surface of a specific tooth on the odontogram.
  • Missing teeth, extractions and root canals: A missing tooth or extraction is indicated with an ‘X’ or a cross through the tooth, while root canals are indicated by a line running down through the tooth’s root. 
  • Restorations: Fillings, crowns and other completed treatments are usually shaded in blue or black to clearly distinguish past treatments from treatment required (which are in red). 
  • Periodontal measurements: A periodontal probe is used to record pocket depths around each tooth in millimeters. Other findings, such as bleeding, mobility, furcation involvement and gingival recession, may also be recorded to help assess and monitor periodontal health.
